Thank you for agreeing to participate in the Wellbeing@Work Index™ survey (Survey).
When you complete the Survey, Deloitte Touche Tohmatsu (Deloitte) will collect personal information about you, including age, gender identity, ethnicity, physical characteristics, and certain health and lifestyle information. Deloitte will use the information in an anonymised form:
- to prepare an assessment of your employer’s workplace wellbeing (Wellbeing Report);
- to provide your employer with an indication of how its workplace wellbeing performs against other similar participating Australian organisations.
Information included in the Wellbeing Report is aggregated to a level that ensures that no specific individuals are identifiable.
Deloitte grants you a non-transferable, limited, non-exclusive, revocable, royalty-free licence to access and use the Survey solely for the purpose of providing your responses. As between you and Deloitte, all materials (including logos, company names, trademarks, images, software and content whether downloaded or not) (Materials) contained in the Wellbeing@Work Index are owned by Deloitte unless indicated otherwise.
